In the boxes below, please list each child that you will be registering with CHAT. Please list the name, sex, date of birth, and grade (as of September 1st) for each child.

CHAT strives to create a safe and fun learning environment for all children.  Please tell us any additional information about your children that you feel we should know.  Please list any allergies, medical needs/equipment (i.e. inhalers, epi-pen), or special needs that apply to any of your children.

I understand that I must agree to and sign the Statement of Faith and Code of Conduct Statement, and that any family may be accepted ordenied for any reason based on the evaluation of the leadership team.

Complete the online registration form, and send in payment for your children's first choice class fees and a non-refundable $145.00.  Payment may be mailed to the post office box listed below, or made via Venmo to @CHAT-Homeschool.  Please make checks payable to CHAT.

You will be considered registered for a class once payment is received.  Any class fee adjustments will be made on registration night.  

The $145.00 fee includes an $80 annual membership fee, $5 background check fee, plus $60 utility fee.  The membership fee and background check are only charged once per school year.  The utility fee is charged each semester of the school year.
