CHAT Registration - Spring 2024

This is the CHAT Registration Form that you will fill out to register all of your children for their CHAT classes. Please scroll all the way to the bottom to add up your registration fees and let us know how you will be paying. This form has space to register up to six (6) children, and then total the fees at the bottom.

Student Registration and Class Fees
When choosing classes, please list the NAME and AGE of your child.  Then, choose a class option for this child for each hour. 

If your child will be in the Nursery, Toddler, or Preschool rooms, please mark the appropriate box and leave the hourly options blank for this child.  

**You are choosing your child's first choice of class for the hour.  It is possible that a class may fill up and your child will need to be enrolled in the other class that is being offered for that hour.  We will create a wait list for classes that fill up quickly.  If a class is no longer listed, please pick another class to enroll in that hour.  All "FULL" classes will be listed in a checkbox section below the hour that they are being offered.  Let us know you in this section that you would like your child to be added to the waitlist for a class.  You may also comment in the "Additional Information" section at the bottom of this form.**

**Membership Fee will not need to be paid if you paid this fee in the fall.

All class fees for your children + $145.00 ($80 annual membership fee, $5 background check fee, and $60 utility fee)

**PLEASE NOTE - If you are a returning family, and paid your membership fee in the fall, you DO NOT include the $85 fee. Only class fees for your children and the $60 utility fee**
